AFME-PUW-FRM-001 PUWER Equipment Assessment Template

A fillable, multi-disciplinary PUWER assessment that brings Engineering, Hygiene, Technical / Food Safety and Health & Safety together on one machine. Each team answers its own questions with evidence, every "No" becomes an action with an owner and date, and all four teams sign a joint decision on whether the equipment is fit for use. A machine can pass an engineering check and still be unsafe to clean, a food safety risk or a danger to people; this template stops anything falling between departments.

Who this resource is for

Engineering leads, hygiene managers and team leaders, technical and food safety managers, and health and safety advisers who jointly decide whether work equipment is fit for use. Also useful for project engineers and engineering managers bringing new or modified equipment into service.

How to use this resource

1. Use it before new or second-hand equipment is first used, after a modification, rebuild or relocation, as a periodic review of high-risk equipment, and after an accident, near miss or enforcement visit. 2. Engineering completes the equipment details and arranges a joint walk-round. 3. Each team completes its own section, ticking Yes, No or N/A and recording the evidence. 4. Every No becomes an action with a risk rating, owner and due date. 5. All four teams agree and sign the final decision. For a whole site, use the asset register tool to track every machine.
